Critical Customer Notification Form

If your life, or the life of a loved one, depends upon electrically-powered medical equipment, such as a ventilator or a dialysis machine, please let us know.  By letting us know, we can place you on our Critical Care Customer Notification List. 

We will make every effort possible to contact customers on the Critical Care Customer Notification List in advance of any planned outages scheduled to affect their residence.

To register as a Critical Care Customer, please complete our Critical Care Customer Notification Form. Please ensure your completed form is accurate and up-to-date. Forms must be signed by your physician prior to submission.
